We accept applications 9:00 am - 4:00 pm Monday through Friday.

Or you can fill out online application form so you do not have to fill out complete applicaiton.

Documents Required

All of the following documents are required in order to begin the interview process:

1. Current Florida State, RN, LPN, CNA, HHA licesnse
2. Current CPR Card
3. Original 4 hour AIDS certificate (update required if older than 2 years)
4. Driver’s License
5. Car Insurance
6. Social Security Card
7. Proof of Citizenship/ work authorization
8. 2 personal references
9. 2 work related references

Hiring Process

1. Preliminary - 2 Face to Face Interviews
2. Exam – need to score 75% (allowed to take twice within 6 month period)
3. Application Completion
4. Florida Department of Law Enforcement Level 1 Screening, Reference Checks, and other screenings as applicable
5. Orientation – internal practices and procedures

Any job offer is contingent upon:

1. Physical (6 months or less)
2. Chest x-ray or PPD
3. Interview/Test Process
4. Favorable Refences and Background checks
